Injury Waiver and Release of Liability

Participation in active physical games carries inherent risks. While many risks can be avoided, it is impossible to guarantee the safety of all participants.

By registering myself or my child in the programs offered through Floor Is Lava Class, I acknowledge and agree to the following:

• I affirm that I, or my child, am in good physical condition and do not suffer from any known disability or condition that would prevent or limit participation.
• I understand that classes may include physical activity such as running, jumping, balancing, and moving across low equipment placed on the floor.
• I acknowledge that enrollment and participation are purely voluntary and not mandated by Floor Is Lava Class.
• In consideration of participation, I hereby release Floor Is Lava Class, its owners, employees, subcontractors, independent contractors, instructors, assistants, and partner schools from any claims, demands, or causes of action related to injuries, illnesses, or other conditions, whether known or unknown, that may occur now or in the future as a result of participation.

By registering, I confirm that I have read and understand this waiver and release, and I agree to its terms on behalf of myself or my child.
